Overview
You can report an incident on any client site you service, from the Forsite Portal. The report goes straight to the client, who investigates it. Your workers can report from the Forsite App instead.
Steps
- Open the site
- Go to Serviced Sites and click the site's name in the Site List.
- Start the report
- Choose Incident Management in the left menu, then click + Report an Incident.
- Classify it
- Pick one of the client's categories under Category, then set Severity and Type. Add a Sub-Type if one fits.
- Answer Injury involved? and Sensitive event?.
- Say what happened
- Write a short Subject and enter the Location and the date and time it Occurred at. You can also pin the spot on the map.
- Choose the Worker type.
- Complete the client's form
- Once every field marked with an asterisk is filled in, the client's incident form opens below. If more than one form fits, choose one.
- Submit
- Click Submit Incident, then Confirm. A message confirms the report, you go back to the site's incident list, and the client's team is notified.
What you can see afterwards
- The site's incident list shows your company's reports for that site, with tiles counting Total Incidents, Open and Injuries.
- The State column changes as the client works through the incident.
- You can't open an incident from the list, edit it or add corrective actions. The client's team runs the investigation.
FAQs
Can I change a report after submitting it?
No. Contact the client with the correction.
Why can't I see a report I submitted?
If you answered Yes to Sensitive event?, only the people the client assigns to the incident can see it, so it won't show in your list.
Will I be told when the client updates or closes it?
You get a notification when the report is created. After that, follow its progress in the State column.
Can my workers report incidents from the App?
Yes. See the guide on reporting a risk in the Forsite App.
