Overview

New incidents land in your incident log, whether they were reported in the Forsite App, by a service provider in the Portal, or by your own team. You work each one from there through to close-out.

Steps

  1. Open the incident log
    • Go to Risk Management, which opens on Incident Log, or open a site and choose Incident Management. Depending on your plan, the tab may be called Incidents & Hazards or Incidents.
    • New reports sit at the top of the list with a light red background. The bell's Incident Alerts tab lists them too.
  2. Open the incident
    • Click its row. If a service provider reported it, their company shows as Involved Company in Step 2.
  3. Switch to Edit mode
    • Click Edit to change Steps 1 to 7, 9 and 10, then click Save Content Changes. Step 8 doesn't need Edit mode.
  4. Move it to the next stage
    • In Edit mode, buttons beside Save Content Changes move the incident to its next stage. Your company's workflow sets their names.
    • The log's State column shows each incident's stage.
  5. Record the investigation
    • Work through Step 3: Hazard Analysis to Step 7: Residual Risk Assessment as they apply, then Step 9: Learnings & Outcomes. Steps 3 and 6 have a button to confirm there's nothing to record.
  6. Add corrective actions
    • In Step 8: Corrective & Preventive Actions, click + Add Action for each follow-up, or Acknowledge: no corrective actions required.
  7. Complete the close-out
    • Fill in Step 10: Review, Approval & Close-out, including Management review and Close-out verification.
  8. Close the incident
    • In Edit mode, use your workflow's closing button. The incident locks and becomes a report you can print with Print / Save as PDF. If your workflow allows reopening, that button shows on the report.

Troubleshooting

  • I can't see Risk Management or Incident Management → Your role may not include incidents. Ask your Company Administrator. If nobody in your company has it, incidents may not be switched on. Contact Forsite support.
  • Rows in the incident log don't open → Opening an incident needs edit access to incidents. Ask your Company Administrator.
  • Incidents only show Step 1 and Step 2 → Your company's plan doesn't include the investigation steps. Contact Forsite support.
  • There are no stage buttons, or a step says you have view-only access → Click Edit first. Your workflow decides who can move an incident and edit each step at each stage, so if the buttons are still missing, ask your administrator.

FAQs

How do I enter an incident myself?
Open the site, choose Incident Management and click + Report an Incident. Fill in the details marked with an asterisk, complete the incident form that appears below them, then click Submit Incident and Confirm.

Why can't I choose Closed in the Status list?
Incidents close through a workflow stage, using the buttons at the top of the incident in Edit mode. If an incident has no workflow, choose one in its Workflow section in Edit mode, or ask your administrator to set one up.

Why isn't every incident in the log?
An incident marked as a sensitive event only shows to its Incident Owner/Manager (Exclusive Access) and Attached Reviewer. Your administrator can also limit which incident categories each team sees.

Who sets up incident forms and stages?
An administrator, under Enterprise Risk Settings. Incident Templates decide which form a reporter fills in, Workflows set the stages, and Auto-Trigger Rules can assign new incidents or put them on a workflow starting at a chosen stage. You only need these if the defaults don't suit you.
